AMSPAR Student Registration Form

Registration for our Level 5 Certificate/Diploma in Primary Care and Health Management  

(Now including our Level 5 Award) 

Congratulations on deciding to register for our Level 5 qualification in Primary Care and Health Management.  The process is easy.  Complete the application form below and send.

 

We will acknowledge receipt and confirm any additional details with you.  You will subsequently receive confirmation of your registration and will be provided with all the necessary information to gain access to our Health Academy and your chosen course.

 

Your application will normally be processed within 7 days. You will be allocated to the chosen starting date.  If this is not possible, you will be advised and you will offered our next available starting date.

 

Please ensure that you attach your supporting documentation and make the appropriate payment.

Payments can be made using online banking (see form for bank details).  Alternatively, you can make telephone card payments by contacting Head Office.  Card payments can also be made using our secure online payments via PayPal.

 

 

All newly registerd student members will receive FREE student membership, which allows you access to some of our membership resources throughout your time of study.  Details about paid professional membership which is available upon completion of the course is shown below the application form.

 

Your route to Professional Membership

Once you have completed your course, you will be able to apply for full membership.

 

Those that complete the Award will be able to apply for full paid membership and this entitles them to the use of the relevant post nominal and designation.  Those working in general practice, prison service, private clinics and the armed forces may use the term "Certified Practice Administrator" and the post nominal CPAdm MAMS AIPC.  This includes membership of the Institute of Primary Care (your chosen faculty).

 

Those students who come from a secondary care or other health organisation background may use the term "Certified Health Administrator" and the post nominal CHAdm MAMS AISC.  This includes membership of the Institute of Secondary Care (your chosen faculty).

 

Those that complete the Certificate will be able to apply for full paid membership and this entitles them to the use of the relevant post nominal and designation.  Those working in general practice, prison service, private clinics and the armed forces may use the term "Certified Practice Manager" and the post nominal CPMgr MAMS AIPC.  This includes membership of the Institute of Primary Care (your chosen faculty).

 

Those students who come from a secondary care or other health organisation background may use the term "Certified Health Manager" and the post nominal CHMgr MAMS AISC.  This includes membership of the Institute of Secondary Care (your chosen faculty).

 

ALL students who successfully complete the Diploma will be entitled to use the designation "Certified Strategic Manager" and the post nominal CSMgr MAMS AIPC or CSMgr MAMS AISC (You choose your preferred faculty membership).

 

Advancement to membership at Fellow level is automatically available providing you have been a fully paid up member for 5 years post qualification.
